If you have an active DSL broadband service with your current or previous service provider, e.g. eircom or Vodafone, please note that this DSL broadband service will automatically be disconnected once your phone number has been transferred to UPC. If you cancel this DSL broadband service yourself this could delay the transfer of your phone number to UPC. Please confirm you understand this.*
If you rely on your existing phone connection for your TV, house alarm or any other device or service, please confirm you are aware that you are responsible for contacting your provider to ensure continuity of service. *
Please confirm you understand that by completing this form you are requesting UPC to transfer your specified home phone number(s) to UPC and that this will result in your current phone service with your current provider coming to an end and possibly the closure of your account with this provider. You authorise your current provider to cease service to your address as necessary in order to successfully transfer your number to UPC. You also confirm that you are authorised to act on behalf of the household in this matter. By using this online form you are not required to sign a contract form.*
